Community Care Worker

Part time – 15 hrs / week

Great Ocean Road Health (Lorne) is seeking to employ Community Care Workers to provide personal care, domestic assistance services and social support to clients in their homes. We are seeking a compassionate, enthusiastic and caring individuals to join the Community Care team.

The position of Community Care Worker requires the following:

  • Current or working towards Certificate III in Individual Support (or equivalent)
  • Apply Advanced First Aid
  • Excellent communication skills
  • Current Drivers Licence.

Registered Nurse – Aged Care (Lorne)

Part time / Fixed Term (3 months)

(48 hrs (0.6 FTE) / fortnight)

Great Ocean Road Health is currently seeking a highly motivated Registered Nurse to join their amazing team. The successful candidate will be committed to the delivery of high quality client focused services and the highest standards of care guided by our values of service and Great Care Framework. This is a fixed term opportunity from November 2020 – February 2021.

  • Current Registration with the NMBA Division 1 [General]
  • A passion for patient centred care and commitment to delivering excellence in clinical care and demonstrated experience in Aged Care setting.

Team Leader in Home Support

Part time (32 hours / week) up to Full time (negotiable)  

A new and exciting opportunity exists for the role of Team Leader within the Community Care Team. The Team Leader In Home Support is responsible for leading the In Home & Social Support team within the Community Services Division, coordinating the day to day operations of the team, including consolidation of documentation and legislative adherence and ensuring individualised services and promoting positive relationships with clients and participants.

The successful candidate will possess;

  • A minimum of a Certificate IV in an appropriate discipline such as Office Administration or Project Management.
  • Demonstrated experience in leading a team to achieve outcomes.
  • Knowledge of the changing environment in the health sector, including funding, supplier market and emerging trends in service delivery. (desirable).

Home Care Packages Case Manager

Part time (32 hrs / week) up to Full time (negotiable)

GORH is offering an exciting career opportunity in a dedicated multidisciplinary team with a dynamic team culture where staff thrive on making a difference. Our Home Care team are seeking an experienced Home Care Package Manager to provide tailored care and support to clients living within the community.

The Role

You will be responsible for:

  • Case Management and Care Coordination
  • Manage and lead quality of service
  • Home care documentation management and accurate processing
  • Effective and efficient communication between home care team, clients and stakeholders
  • Compliance with Regulatory and Accreditation standards
  • Delivery of services and care in accordance with Customer Agreements
  • Developing and maintaining knowledge and skills to support ongoing success
  • Viability and operational effectiveness of your customer case load
  • Developing, implementing and maintaining cost management strategies
  • Internal auditing and reporting is completed in a timely manner
  • Supervise Home Care Package Coordinator and Administration Officer
  • Role model for a positive workplace culture at GORH.

Skills and Experience

  • Degree or Diploma level qualification in Aged Care/Community Care/Social Welfare/Social Sciences or relevant field
  • Demonstrated experience in provision of case management and home based assessments
  • Proven ability to negotiate, manage and operate within a budget
  • Current Victorian Driver’s Licence and own reliable vehicle with comprehensive insurance
  • Must be flexible and self-motivated with excellent communication skills and a genuine care for older people
  • Current valid Police Check and a Valid Australian Working Visa are required prior to employment
  • Flexibility and willingness to work autonomously and as part of service delivery of the Community Services Division.

General Practitioner – Apollo Bay Medical Centre

Full-time and part-time positions available

POSITION DESCRIPTION

  • Busy and growing, mixed billing clinic – high earning potential
  • Ability to work  part-time or full-time
  • Desirable location on the Great Ocean Road
  • Experience or interest in the provision of supervision for GP Trainees


ROLE

This GPA accredited mixed billing clinic sees 120-160 patients a week, providing care for the local communities as well as a significant tourist population between December and April.

As Apollo Bay Medical Centre is part of Great Ocean Road Health, there is an integrated multidisciplinary approach to care within the health service and a sharing of staff and expertise with Lorne Medical Centre, also part of Great Ocean Road Health. The service is also well supported by specialist regional services from Barwon Health

Participation in the After-Hours Urgent Care on call roster is available.

Support from a Practice Nurse, experienced Administration staff and a Practice Manager, will see you rapidly transition smoothly into the community within this well-established Centre.

About Apollo Bay Medical Centre

PRACTICE DETAILS

The Apollo Bay Medical Centre is integrated within the Otway Health site, part of Great Ocean Road Health, which includes Urgent Care, Aged Care (20 beds) Acute Care and Community Care. The centre has visiting Allied Health and Specialist services on a regular basis. Pathology services are also on site with a pharmacy located within the community.

Apollo Bay Medical Centre currently has 3 doctors – 1 VR, 2 Registrars and Nurse Practitioners in the Urgent Care.  The Medical Centre consists of 5 consulting rooms with a treatment room.

Current practice hours are Monday to Friday, 8:45 am – 5.45 pm and Saturday 9 am to 12 pm, closed on Sundays.

The main patient health issues impacting on the practice are women’s health, Diabetes, and Chronic Disease Management within the community, with a wide range of clients accessing services during the summer months with an influx of tourists to the region between December and April.

Access to an Allied Health referral network is available through Otway Health with a wide range of services readily accessible.

Lorne and Apollo Bay practices are operating in similar organisation structures with integrated primary care focus. They work together to provide Great Care to the communities along the Great Ocean Road.
