Great Ocean Road Health’s Personal Care Service can assist you to undertake the activities of daily living, to help you maintain your independence and continue to live at home. For more information, please contact us on 03 5237 8575.
HOW IT WORKS
Once registered with My Aged Care, a Living at Home Assessment will be carried out by a Regional Assessment Officer where your needs are documented and a Care Plan is developed with you, your family member, or advocate.
Your Care Plan will include details of the CHSP funded Personal Care Services which Otway Health will deliver to assist you undertake the activities of daily living.
We work with you to undertake some of the tasks you may be finding difficult to complete. By working alongside you, we aim to assist you to maintain your independence, allowing you to continue to live independently at home.
The regularity of this service, as well as fees and tasks to be undertaken are determined with you during your Living at Home Assessment.
SERVICES WE OFFER
Services that we can assist you with include:
- showering
- medication prompting
- grooming (such as shaving)
- dressing and undressing
- meal preparation
- attendance to pay accounts
- escorting to medical or specialist appointments
- preparing a shopping list
NON-PERSONAL CARE SERVICES

- administering medication
- wound care
- blood pressure monitoring
- catheter care
- nail care
