Great Ocean Road Health’s Respite Care service provides you with quality care and companionship whilst your primary care giver has a break from their caring responsibilities.
HOW IT WORKS
Once registered with My Aged Care, a Living at Home Assessment will be carried out by a Regional Assessment Officer where your needs are documented and a Care Plan is developed with you, your family member, or advocate.
Your Care Plan may include details of the CHSP funded Respite Care service which Great Ocean Road Health will deliver to eligible persons who fulfill the role of primary care giver. When this happens, our staff can look after you whilst your primary care giver has a break from their caring responsibilities.
The regularity of this service, as well as fees and agreed respite activities are determined with you during your Living At Home Assessment.
HOW IS RESPITE CARE OFFERED?
Respite Care services are offered both in and out of your home.
Out of home respite care may involve a day trip or half day excursion to a local attraction, cafe or an organised community activity.
During respite visits, we aim to maintain your independence as much as possible and so, information regarding your favourite activities and interests are sought during your assessment.
AVAILABILITY
Monday – Friday excluding public holidays.
Respite Care can be provided weekly, fortnightly, monthly or irregularly, to best suit your situation.
