Great Social Support Groups

Great Ocean Road Health’s Social Support Groups are designed to encourage community members to participate in healthy, recreational and cultural activities that stimulate wellbeing.

HOW IT WORKS

Once registered with My Aged Care, a Living at Home Assessment will be carried out by a Regional Assessment Officer where your needs are documented and a Care Plan is developed with you, your family member, or advocate.

Your Care Plan will include details of the CHSP funded Social Support Group services which Great Ocean Road Health will deliver, so that you can participate in social activities.

ACTIVITIES WE OFFER

A range of activities are offered to ensure variety and stimulation. You will have the opportunity to socialise, undertake gentle exercise, and improve and maintain your emotional wellbeing and cognitive ability.

Activities and outings are planned by group members and are designed to assist you to maintain your independence and self-reliance whilst exploring new and old areas of interest.
