Great Ocean Road Health’s Social Support – Individual is designed to assist you to participate in community life and feel socially connected to your community.

HOW IT WORKS

Once registered with My Aged Care, an assessment will be carried out by a Service Assessment Officer where your needs are documented and a Care Plan is developed with you, your family member, or advocate.

Your Care Plan will include details of services which Great Ocean Road Health can deliver to assist with your independence and every day living.

ACTIVITIES WE OFFER

A range of activities is offered to ensure variety and stimulation. You will have the opportunity to engage socially in the community and improve and maintain your emotional wellbeing.

Outings are determined by yourself as the participant and are to assist you to maintain your independence and self-reliance whilst exploring new and old areas of interest. Individual social activities include:

• Accompanied shopping/bill-paying
• Going to lunches or coffee
• Cooking together
• Attending appointments
• Telephone monitoring service
